Ces besoins peuvent \u00eatre&nbsp;:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>l\u2019attribution de mat\u00e9riel p\u00e9dagogique adapt\u00e9,<\/li>\n\n\n\n<li>l\u2019accompagnement humain,<\/li>\n\n\n\n<li>la dispense d\u2019un ou plusieurs enseignements.<\/li>\n<\/ul>\n\n\n\n<p>Le PPS suit l\u2019\u00e9l\u00e8ve durant toute sa scolarit\u00e9 et s\u2019impose \u00e0 tous les \u00e9tablissements (scolaires, sanitaires et m\u00e9dico-sociaux).<\/p>\n\n\n\n<p>&#8211; Les documents de mise en oeuvre du PPS (outils d\u2019observation des besoins de l\u2019\u00e9l\u00e8ve en classe, adaptations propos\u00e9es\u2026)<\/p>\n\n\n\n<p><strong>Les documents de mise en \u0153uvre du PPS<\/strong>, qui figurent en annexe de la&nbsp;<a href=\"http:\/\/scolaritepartenariat.chez-alice.fr\/page229.htm\">circulaire du 8 ao\u00fbt 2016<\/a><strong>&nbsp;liste de gestes professionnels&nbsp;<\/strong>pour scolariser un \u00e9l\u00e8ve en situation de handicap<\/p>\n\n\n\n<p>\u25ba&nbsp;<a href=\"http:\/\/cache.media.education.gouv.fr\/file\/30\/14\/1\/ensel034_annexe1_616141.pdf\">Projet personnalis\u00e9 de scolarisation&nbsp;: Document de mise en \u0153uvre \u2013 \u00e9cole maternelle<\/a><br>\u25ba&nbsp;<a href=\"http:\/\/cache.media.education.gouv.fr\/file\/30\/14\/3\/ensel034_annexe2_616143.pdf\">Projet personnalis\u00e9 de scolarisation&nbsp;: Document de mise en \u0153uvre \u2013 \u00e9cole \u00e9l\u00e9mentaire<\/a><br>\u25ba&nbsp;<a href=\"http:\/\/cache.media.education.gouv.fr\/file\/30\/14\/5\/ensel034_annexe3_616145.pdf\">Projet personnalis\u00e9 de scolarisation&nbsp;: Document de mise en \u0153uvre \u2013 second degr\u00e9<\/a><br>\u25ba&nbsp;<a href=\"http:\/\/eduscol.education.fr\/cid96837\/gestes-professionnels-adaptes-pour-scolariser-un-eleve-en-situation-de-handicap.html\">Liste de gestes professionnels<\/a><\/p>\n\n\n\n<p>-Les \u00e9valuations de l\u2019\u00e9l\u00e8ves et les \u00e9l\u00e9ments permettant la mise en place de son parcours scolaire (parcours avenir notamment pour le coll\u00e8ge)<\/p>\n\n\n\n<p>-Tous les documents concernant les prises en charge ext\u00e9rieures permettant la mise en \u0153uvre de son PPS.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">L\u2019adaptation des objectifs d\u2019apprentissage<\/h4>\n\n\n\n<p>La circulaire du 8 ao\u00fbt 2016 indique que quand la CDPH a pr\u00e9conis\u00e9 une adaptation des objectifs d\u2019apprentissage, ce sont les enseignants qui doivent construire et formaliser cette programmation adapt\u00e9e des objectifs d\u2019apprentissage, qui sera propos\u00e9e en ESS.
